Cardiovascular Health Awareness Program

Older adults and those at risk for future cardiovascular disease or other chronic diseases are invited to take part in the Cardiovascular Health Awareness Program (CHAP) by attending free blood pressure measurement and monitoring sessions in Stratford.

 

The goal of CHAP is to PROMOTE cardiovascular health in our community and to raise awareness about the importance of blood pressure monitoring. Trained volunteers will help you:

  • Measure your blood pressure and record your results.
  • Complete a checklist about risk factors for heart disease and stroke.
  • Address modifiable risk factors.

A copy of your results will be given to you and with your permission, sent to your family physician and regular pharmacist.

The Cardiovascular Health Awareness Program (CHAP) was designed to support prevention of cardiovascular disease (CVD) and stroke in communities.  The program relied on the participation of family physicians, pharmacists, volunteers, community organization and older adults.  Older adult residents were invited to attend volunteer-led sessions in local pharmacies for a cardiovascular risk assessment including blood pressure monitoring.  Participants took home a copy of their cardiovascular disease and stroke risk profile, as well as resources on modifiable risk factors.  Program results were sent to each participant's family physician and regular pharmacist to enhance care. 

The aim of CHAP is to create a sustainable model of CHAP for long-term implementation in communities, while also expanding several components of the program.  CHAP targets older adults at risk for future CVD and stroke morbidity and mortality.  Volunteer peer health educators will take on an expanded role with participants both inside and outside of CHAP sessions.  At the sessions, volunteers will assist participants to complete and follow the web-based Heart & Stroke Foundation Blood Pressure Action Plan, a tool that generates an individualized plan to improve modifiable risk factors.  Group education sessions will also be held to improve self-management of CVD.  The program will also link with the new Ontario pharmacist consultation program, MedsCheck.

1st Tuesday of the month Shoppers Drug Mart, 211 Ontario St.
2nd Monday of the month Kiwanis Centre **until 12:00 pm
3rd Wednesday of the month Tavistock Health Clinic
3rd Friday of the month Pharmacy at Jenny Trout, 342 Erie St.
4th Friday of the month West End Pharmacy, 150 Huron St.

All sessions take place from 9:30 am to 1:00 pm, except where noted.

Volunteer Opportunity: CHAP Peer Educators

Volunteers help CHAP clients by taking their blood pressure, recording results and providing education based on their cardiovascular risk profile.

Time commitment is four hours per month.
